The painting is so titled because it seems that the water comes out of the left side and leaves out the right side. Water seems to be coming out of everywhere in this painting.
The light seems to sneak into this dark grotto by reflecting on from the water, the rocks or off other things within the scene. The bright greens in the water are from lit up leaves overhead outside the scene. I paid special attention to set up a level of complementary color contrast between the green mixtures needed for the sunlit areas and the shadow areas.
